Step 2: Additional Items
Deliver to Office
to complete registration- Original State Birth Certificate. A copy is made and you keep original.
- Copy of current Report Card & Standardized Test Scores.
- Alabama Certificate of Immunization (Must be the current shot record not an exemption)
- Transcripts. SJS will contact previous school.
